Typo fixes, changes, and readme improvement 

- Update CHANGELOG.md
Fixed mistakes, added reference to an issue and went more verbose on how something was fixed.

- Update README.md
Attempt to improve readability and added some detail to make it hopefully easier to understand.

- Typo fixes in proc.c
